Job summary

Prairie Community Services in Morris, MN is seeking a Direct Support Provider for a part-time, on-site role at Woodland Home. You will assist residents with daily living tasks, personal care, medication administration, and community activities, while maintaining daily charting.

Starting wage is 16.50–17.50 per hour. Candidates must be 17+, have a valid DHS background clearance, a Driver's license, and strong English communication skills. We offer comprehensive benefits and flexible scheduling.

Qualifications

• Must be 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver's license.
• Must be able to read, write, communicate and understand the English language while conducting company business.

Responsibilities

• Provide training and assistance with daily personal cares.
• Administer medications as required.
• Coordinate community activities and money management.
• Prepare meals, do laundry and clean duties.
• Maintain daily charting and documentation.
