Job summary

Take Another Chance is looking for a dedicated Direct Support Professional to join our Ohio team in a 24-hour awake residential setting. The role focuses on daily living assistance, medication administration, transportation, and community integration while implementing individualized support plans.

No prior DSP experience is required; we provide paid training and certification support. This part-time position offers competitive pay between $16.00 and $18.00 per hour, with pay based on experience

Qualifications

• Must be at least 18 years old.
• High school diploma or equivalent.
• Valid Ohio Driver’s License with Ohio BMV.
• Reliable daily access to a personal vehicle to transport clients.
• Proof of active auto insurance meeting Ohio minimum requirements.
• Ability to pass both a criminal and a driving history background checks.
• Ability to lift up to 50 pounds and perform physical tasks.
• Willingness to complete required training and certifications provided by the company.

Responsibilities

• Provide person-centered support with activities of daily living, including bathing, grooming, toileting, dressing, and meal preparation.
• Administer medications in accordance with agency policies and Ohio DODD requirements after completing required training and certifications.
• Support individuals in community activities, appointments, transportation, and personal interests.
• Complete required documentation, including daily notes, medication records, and incident reports.
• Build positive relationships with individuals served, families, guardians, and team members.
