Oak Hill is seeking a Direct Support Professional in Glastonbury, CT to assist individuals with daily living, health, and community engagement. Expect a 40-hour week, Mon-Fri 6am-2pm, and a base rate of $20.39/hour with differential pay for specific schedules.

You’ll document health information in the EHR, administer medications per directives, and transport clients as needed. Training provided; growth opportunities and a strong benefits package are included.

Formación

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver’s license to transport individuals in agency vehicles.
• Ability to pass required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional staying beyond scheduled shift for coverage.

Responsabilidades

• Assist with daily living activities (personal care, grooming, hygiene) according to plans.
• Monitor health information and document vital signs when required.
• Administer medications and maintain medication records.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and assist with mealtimes.
• Support mobility and transfers using approved equipment and techniques.
• Document daily activities, health info, progress notes, incidents in EHR.
• Follow emergency procedures and safety protocols; apply behavioral support if needed.
