Oak Hill in Glastonbury, CT is seeking a Direct Support Professional for Community Programs. You will support individuals with developmental and intellectual disabilities to live healthy, meaningful lives. Shift: Saturdays and Sundays 7:00 AM–3:00 PM, 16 hours weekly.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Support individuals in living healthy, meaningful, and fulfilling lives by assisting with daily living activities.
• Provide continence care as needed and encourage independence and skill development.
• Monitor and document health information, assist with medications as trained, and follow health care plans.
• Transport individuals to appointments and community activities using agency vehicles and participate in community life.
• Prepare meals and snacks according to menus and personal preferences, assist during meals.
• Assist with mobility and transfers using authorized techniques and equipment.
• Document daily activities, health information, progress notes, incidents, and medication administration in the EHR, following regulatory requirements.
• Respond to emergencies and behavioral needs using trained procedures and First Aid/CPR when required.
• Communicate effectively with team members and participate in trainings and development.
