Job summary

Oak Hill is seeking a Direct Support Professional in Windsor, CT, to help individuals live healthy, meaningful lives. 20 hours per week, Monday-Friday 6:00 am - 10:00 am. Base pay $20.39/hour with differential pay for specific schedules.

Role involves daily living support, health monitoring, transportation, meals, and documentation. Male applicants only. Union, non-exempt, overtime eligible.

Qualifications

• Must be at least 18 years old and have a high school diploma or GED.
• Valid driver's license and ability to transport individuals in agency vehicles.
• Ability to complete required background checks and health screenings.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Monitor health information and document health data as required.
• Administer medications and maintain medication records per policies.
• Transport individuals to appointments and community activities.
• Prepare meals and snacks per menus and dietary needs.
• Assist with mobility and transfers using approved equipment.
• Document daily activities and health information in the EHR.
• Follow emergency procedures and safety protocols.
• Communicate effectively with team, individuals served, and families.
