Job summary

The Center for Disability Services in the Capital Region seeks a Direct Support Professional II - Residential Services to support individuals in their daily living activities and treatment plans. Responsibilities include participating in planning and interdisciplinary teams, promoting independence, and helping with Personal habilitation per care plans.

A NYS driver's license is required; 18+ with lifting ability and high school diploma are required; 1 year of related experience preferred.

Qualifications

• NYS Driver's License required.
• High School diploma or equivalent.
• Must be at least 18 years old and able to lift a minimum of 50 pounds.
• One (1) year of related experience preferred.

Responsibilities

• Serve as a member of the person’s planning, implementation, and interdisciplinary teams.
• Assisting, promoting, and providing independence in the daily treatment, care, and services of the people we support.
• Respect the person’s choices, lifestyle, and their dignity to take informed risks.
• Help implement personalized habilitation services, as outlined in their personalized care plan.
• Assist in acquisition of skills, including basic activities of daily living, self-care and household management for the people we support.
