The Center for Disability Services seeks a Direct Support Professional II to support individuals in residential settings across the Capital Region. The role focuses on implementing personalized care plans, promoting independence, and collaborating with interdisciplinary teams to enhance daily living skills and quality of life.

Qualifications

• NYS Driver's License required.
• High School diploma or equivalent.
• 1 year of related experience preferred.
• Must be at least 18 years old and able to lift 50 pounds.

Responsibilities

• Serve as a member of the person’s planning, implementation, and interdisciplinary teams.
• Assisting, promoting, and providing independence in daily treatment, care, and services.
• Respect the person’s choices, lifestyle, and their dignity to take informed risks.
• Help implement personalized habilitation services per their care plan.
• Assist in acquisition of skills including basic activities of daily living, self-care and household management.
