Job summary

OneWell Health Care in Newtown, PA is seeking a dedicated Caregiver / Direct Support Professional. This field-based DSP will deliver hands-on, person-centered support to individuals with IDD in home and community settings, aiding daily living, mobility, and community participation.

You will follow individualized service plans and work with an interdisciplinary team to promote independence and well-being. The role requires a high school diploma and offers flexible scheduling within a non-exempt

Qualifications

• High school diploma or equivalent.
• Prior caregiving, home health, or DSP experience preferred.
• Experience with IDD is a plus.

Responsibilities

• Provide hands-on, person-centered support per ISP/IPP.
• Assist with daily living activities, mobility, and meal preparation.
• Monitor health and safety, document observations.
• Support transportation to appointments and community activities.
• Maintain accurate, timely documentation and HIPAA compliance.
• Participate in trainings and team meetings.
