Job summary

Oak Hill in Oxford, CT is seeking a Direct Support Professional to help individuals live healthy, meaningful lives. The role involves personal care, health monitoring, mobility assistance, and transportation within a Fri-Mon 7am-3pm schedule.

You will document activities in the EHR, follow individualized health care plans, and collaborate with a dedicated team to ensure safety and quality of life for those served.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver’s license to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional stay beyond scheduled shift.

Responsibilities

• Assist with daily living activities per individual care plans.
• Monitor health information and document vital signs as required.
• Administer medications and maintain accurate medication records.
• Transport individuals to appointments and activities using agency vehicles.
• Provide nutrition and mealtime support and assist with mobility and transfers.
• Document daily activities, health information, and incidents in the EHR.
• Follow emergency procedures and apply First Aid/ CPR when needed.
• Communicate effectively with team, individuals served, and family members.
