Job summary

Sevita is seeking a Direct Support Professional to provide direct supports in a residential setting. You will work within a single program to assist individuals with daily living activities, health maintenance, and skill development, following individualized service plans.

The role requires empathy, reliability, and the ability to document progress and maintain confidential records. A valid driver's license is required, and some shifts may include transportation of program participants.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age and have working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Maintains healthy and professional relationships with individuals, families, guardians and case managers; complies with Sevita Customer Service Standards.
• Reports any instance of abuse/neglect and incidents per policy; ensures confidentiality and safety.
• Supports safety and rights through universal precautions and proper documentation.

Skills

Six months of experience in human serv

Working knowledge of computers

Education

High School Diploma or equivalent

Valid driver’s license

Criminal background checks as required
