Job summary

Sevita is seeking a Direct Support Professional to provide daily living assistance in residential settings, supporting 4–6 individuals per program. Responsibilities include socialization, health maintenance, and documentation, with emphasis on dignity and rights of individuals served.

Role requires high school education (or equivalent), 18+ years old, and at least 6 months of human services experience. A valid driver’s license and computer basics are required.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Makes cost-effective and appropriate purchases within the budget of the individual served.
• Maintains healthy and professional relationships with individuals, families, guardians and case managers; greets family members and other visitors; implements Sevita Customer Service Standards.
• Reports any instance of alleged abuse or neglect; reports medical, behavioral and other incidents in accordance with Sevita policy and external requirements.
• Maintains confidentiality, respects the rights of persons being served; assists individuals in exercising their rights.
• May accompany individuals to medical appointments; relay orders and information to and from medical providers as required.
• Correctly follows procedures in utilizing medical equipment; reports malfunction or disrepair.
• Prepares or supports individuals to prepare food in accordance with planned menus and individual choice; monitors mealtime and correctly implements any dining plans.

Skills
