Job summary

Sevita seeks a Direct Support Professional to provide person-centered support in a residential setting, assisting with daily living activities, transportation, and community integration. You will help individuals with personal care, recreation, and daily routines, while documenting progress and ensuring confidentiality.

The role requires a high school diploma or equivalent, 18 years of age, and a valid driver’s license.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Direct Support: assist with socialization and behavioral development per service plans.
• Record progress and maintain confidentiality.
• Manage assets within budget of the individual served.
• Maintain relationships with individuals, families, guardians and case managers; follow Sevita customer service standards.
• Report abuse or neglect; report medical and other incidents per policy.
• Respect rights and confidentiality; follow the bill of rights.
• Accompany to medical appointments as needed; relay orders to providers.
• Assist with medication self-administration if assigned and document delivery.
• Meal preparation and monitoring of mealtime and dietary plans.
• Adhere to safety policies; participate in safety trainings.
• Attend orientation and monthly staff meetings.
• Maintain punctuality and coverage; work as a team.
• Drive for transportation if required and observe safety.

Education

High school diploma or equivalent preferred

Six months of experience in human services

Must be 18 years of age

Working knowledge of computers
