Job summary

Sevita is seeking a Direct Support Professional to empower individuals with intellectual and developmental disabilities in residential settings. You will provide daily living support, assist with health needs, and help with community integration in accordance with individual plans.

Responsibilities include documentation, safe transport when needed, and collaboration with nurses and supervisors to ensure quality care.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.
• Valid driver’s license in good standing.
• Car registration and vehicle insurance if providing transportation for individuals.
• All state-required trainings and certifications completed in mandated timeframes.

Responsibilities

• Deliver support per individual service plans, including socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, recreation, transportation, and shopping.
• Accurately document progress and activity; maintain confidentiality.
• Make cost-effective purchases within the budget of the individual served.
• Maintain professional relationships with individuals, families, guardians and case managers; follow Sevita standards.
• Report abuse or neglect; report medical and behavioral incidents as required.
• Maintain rights and safety; follow safety policies and training.

Education

High school diploma or equivalent

Six months experience in human services

Must be 18 years old

Working knowledge of computers

Valid driver’s license in good standing
