Job summary

Sevita is seeking a Direct Support Professional to provide compassionate assistance in residential settings. You will work on individual service plans, helping with daily activities, health needs, and community integration.

Responsibilities include documentation, teamwork with staff, and safeguarding the rights of individuals served. A valid driver’s license and background checks are required.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age and able to meet driving requirements.

Responsibilities

• Directly support individuals per service plans, including socialization, health maintenance, and skill/behavioral development.
• Assist with daily activities such as personal care, housekeeping, transportation, community integration, and shopping.
• Accurately document progress and maintain confidentiality; review records and logs as applicable.
• Maintain professional relationships with individuals, families, guardians, and case managers; follow Sevita Customer Service Standards.
• Report any abuse, neglect, or incidents per policies and external requirements; enforce rights and universal precautions.
