Job summary

Oak Hill in Southington, CT is seeking a Direct Support Professional. The role supports individuals with daily living, health monitoring, and community participation. Expect 32 hours weekly with a 2pm-11pm shift on Fri, Sat-Mon.

Overtime eligible, union position, with shift differentials based on assignment. Responsibilities include personal care, medication records, mobility assistance, meal support, and documentation in the electronic health record.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Able to complete background checks, fingerprinting, and screenings.
• Complete required training and certifications; follow safety policies.
• Commitment to safe, healthy, professional work environment.
• Occasional shift coverage beyond scheduled hours.

Responsibilities

• Assist with personal care, grooming, bathing, dressing, and hygiene needs.
• Monitor health information and medications; document in EHR.
• Transport individuals to appointments and activities using agency vehicles.
• Support meals; follow menus and dietary needs.
• Assist with mobility and transfers using approved techniques.
• Follow emergency procedures; apply First Aid/CPR when needed.
• Communicate effectively with team and stakeholders; participate in trainings.
