Job summary

Oak Hill is seeking a Direct Support Professional in Middlebury, CT, to support individuals in living healthy, meaningful lives. The role runs Tuesday through Saturday, 7:00 AM to 3:00 PM, totaling 40 hours weekly, with a base pay of $20.39 per hour and differential pay for specific schedules.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license enabling transport in agency vehicles.
• Ability to complete background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain certifications.
• Commitment to a safe, healthy, professional work environment.
• Occasional shift coverage beyond scheduled times may be required.

Responsibilities

• Support individuals with daily living activities (personal care, grooming, hygiene).
• Assist with health and wellness, monitor vital signs, document health information, and administer medications as trained.
• Transport individuals to appointments and community activities.
• Provide nutrition and mealtime support according to menus and dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, progress notes, incidents, and medication administration in the EHR.
• Follow emergency procedures and safety protocols; apply behavioral support and First Aid/CPR when required.
• Communicate effectively with team members, individuals served, families, and other stakeholders.
