Job summary

WellLife Network is seeking a Direct Support Professional in New York to provide compassionate, person-centered support to adults with developmental disabilities. You will assist with daily living activities, mobility, health needs, and community engagement while documenting progress and collaborating with a multidisciplinary team.

The role requires a high school diploma, fluent English, and a NY driver’s license with driving experience.

Qualifications

• High School Diploma or equivalency diploma required.
• Fluent in English for client communication.
• Valid New York State Driver's License with at least 2 years of driving experience (required for driving positions).
• Experience supporting adults with intellectual/developmental disabilities or behavioral health needs preferred.

Responsibilities

• Provide hands-on care, encouragement, and advocacy to individuals supported.
• Assist with daily living activities, meals, and personal care.
• Administer medications as directed and support medical appointments.
• Assist with mobility and safe navigation of environments.
• Plan and participate in recreational, educational, and social activities.
• Document daily notes, progress reports, and incident reports in electronic systems.
