Job summary

WellLife Network is seeking a Direct Support Professional in New York to provide hands-on care, encouragement, and advocacy for individuals with developmental disabilities in homes and communities. The role emphasizes personal care, health support, mobility assistance, and community engagement, with documentation of progress and collaboration with care teams.

Requirements include a High School Diploma or equivalency, fluent English, and a NY driver’s license with at least 2 years of driving

Qualifications

• High School Diploma or equivalency is required.
• Fluent in English for client communication.
• Valid New York State Driver's License with at least 2 years of driving experience.
• Experience with adults with intellectual/developmental disabilities or behavioral health needs is preferred but not required.

Responsibilities

• Provide hands-on personal care and daily living support.
• Assist with meals, household tasks, and maintaining a safe home environment.
• Administer medications as directed and accompany medical appointments.
• Support mobility and safe navigation with mobility aids and transfers.
• Plan and participate in community, educational, and recreational activities.
• Document daily notes, progress reports, and incidents in electronic systems.
