Job summary

WellLife Network in Bronx, NY is seeking a Direct Support Professional to assist individuals with daily living, health needs, and community participation in their homes and communities. You’ll provide personal care, mobility support, medication administration, advocacy, and documentation, while promoting independence and dignity.

Qualifications include a High School Diploma or equivalent, fluent English, and a NY driver’s license with driving experience.

Responsibilities

• Assist with bathing, dressing, grooming, and toileting.
• Help with meal preparation, household chores, and creating a safe, comfortable home environment.
• Administer medications as directed.
• Support individuals in medical appointments and following treatment plans.
• Assist with walking, transfers, and mobility aids.
• Plan and participate in recreational, educational, and social activities.
• Advocate for needs, rights, and access to services.
• Maintain accurate daily notes and progress reports in electronic systems.
