Job summary

OneWell Health Care is seeking a dedicated Caregiver / Direct Support Professional (DSP) in New Wilmington, PA. This field-based role provides hands-on, person-centered support to individuals with IDD in home and community settings, focusing on daily living activities, health monitoring, and community inclusion.

You will work under supervision with flexibility in scheduling. Qualifications include a high school diploma or equivalent; prior caregiving experience is preferred.

Qualifications

• High school diploma or equivalent.
• Prior experience in caregiving, home health, or direct support for individuals with IDD.
• Experience working in community-based or residential support settings.

Responsibilities

• Provide hands-on, person-centered support in home and community-based settings.
• Assist with daily living activities, meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
• Maintain structured routines, support employment or vocational activities, and assist with transportation to appointments or community outings.
• Document accurately and timely; ensure HIPAA compliance and regulatory standards.
