Job summary

Oak Hill in Monroe, CT is seeking a Direct Support Professional to help individuals live healthy, meaningful lives. You will assist with daily living activities, monitor health, and promote community inclusion.

Responsibilities include documenting health information in the EHR, following care plans, administering medications as trained, transporting individuals to activities, and working with a compassionate team. Schedule: Friday–Monday, 7:00 AM–3:00 PM, 32 hours/week, $20.39/hour base.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Assisting with daily living activities, grooming, bathing, dressing, and hygiene needs.
• Monitoring health-related information and maintaining medication records as trained.
• Transporting individuals to appointments and community activities using agency vehicles.
• Preparing meals and snacks according to menus and dietary requirements.
• Assisting with mobility and transfers using approved techniques and equipment.
• Accurately documenting daily activities, health information, progress notes, incidents, and medication administration in the EHR.
• Following emergency procedures and safety protocols, including First Aid/CPR when required.
• Communicating effectively with colleagues, individuals served, and family members.
