Job summary

Oak Hill in Bristol, CT is seeking a Direct Support Professional to help individuals live healthy, meaningful lives through daily living support, health monitoring, and community involvement.

This on-site role offers a 32-hour workweek with Friday through Monday shifts (3pm–11pm). Paid training, safety protocols, and required certifications are provided to support your success in a mission-driven environment.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver\'s license to transport individuals in agency vehicles.
• Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional stay beyond scheduled shift.

Responsibilities

• Assist with daily living activities including personal care, grooming, bathing, dressing, and hygiene.
• Monitor health information and document vital signs as required; administer medications per training.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and provide mealtime support per menus and dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, and medication administration in the EHR; ensure regulatory compliance.
• Respond to emergencies with established procedures and apply First Aid/CPR when needed.
• Collaborate with team, communicate with staff, individuals served, families, and other stakeholders.
