Qualifications

• Must be at least 18 years old.
• High school diploma or GED required.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Successful completion of required background checks, fingerprinting, and pre-employment health screenings.
• Completion of required training programs and maintenance of necessary certifications.

• Commitment to continuity of care, including occasional shifts beyond scheduled coverage.

Responsibilities

• Assist with daily living activities, grooming, bathing, dressing, and hygiene.
• Monitor health information and document data; administer medications as trained.
• Transport individuals to appointments and community activities in agency vehicles.
• Prepare meals and provide mealtime support per menus and dietary needs.
• Document daily activities and health information in the EHR; maintain compliance with regulations.
