Job summary

Oak Hill is seeking a Direct Support Professional in Oxford, CT to aid individuals with daily living, health monitoring, and community participation. This non-exempt role offers a 28-hour weekly schedule with Sunday, Monday, Friday, and Saturday shifts and a base rate of $20.39 per hour, with differential pay for certain schedules.

You will assist with personal care, medications, documentation in the EHR, and transport to activities, while maintaining a safe, compassionate environment.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED required.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks and health screenings.
• Ability to complete required training programs and maintain certifications.
• Commitment to safety and professional conduct as per Oak Hill policies.
• Willingness to stay beyond scheduled shift for coverage when needed.

Responsibilities

• Assist with personal care, grooming, hygiene, and independence support.
• Monitor and document health information, administer medications per directives.
• Transport individuals to appointments and community activities.
• Prepare meals and assist during mealtimes per dietary needs.
• Assist with mobility and transfers using approved techniques and equipment.
• Accurately document daily activities and health data in EHR.
• Follow emergency procedures and provide First Aid/CPR when required.
• Collaborate with team and participate in trainings and meetings.
