Job summary

Oak Hill is seeking a Direct Support Professional to assist individuals in Mansfield, CT, with daily living, health monitoring, nutrition, mobility, and community participation. This part-time role involves 16 hours weekly with Sat/Sun shifts and a base hourly rate with differential pay for specific schedules, focusing on safety and compassionate care.

You will document health information in the EHR, follow care plans, transport clients, and participate in ongoing trainings to support

Qualifications

Must be at least 18 years of age.
High school diploma or GED.
Valid driver's license with the ability to transport individuals in agency vehicles.
Ability to successfully complete required background checks, fingerprinting, and pre-employment health screenings.
Ability to complete required training programs and maintain necessary certifications.
Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

Assisting with personal care, grooming, bathing, dressing, and hygiene needs.
Providing assistance with continence care as needed.
Encouraging independence and skill development.
Monitoring and documenting health-related information, including vital signs.
Administering medications and maintaining medication records.
Following health care plans and supporting well-being.
Transporting individuals to appointments and community events.
Preparing meals and snacks per menus and dietary needs.
Assisting with mobility and transfers using approved techniques.
Documenting daily activities and medication administration in EHR.
Following emergency procedures and safety protocols; administer First Aid/CPR when required.
Communicating with colleagues and participating in trainings.
