Job summary

Sevita is seeking Direct Support Professionals to provide person-centered services in residential settings. You will support 4-6 individuals, assist with daily living tasks, health maintenance, and community integration, following individual service plans.

Responsibilities include documenting progress, coordinating with nurses and supervisors, and ensuring safety and confidentiality while promoting independence and positive outcomes for those served.

Qualifications

• Basic computer skills
• High school diploma or equivalent preferred
• Six months of experience in human services preferred
• Must be 18 years of age

Responsibilities

• Direct support per individual service plans, including socialization and daily living activities.
• Accurately document progress and maintain confidentiality.
• Manage purchases within the individual’s budget and preferences.
• Maintain professional relationships with individuals, families, guardians and case managers.
• Report any abuse, incidents, and health concerns in line with policy.
• Respect residents' rights and assist with health directives.
• Assist with medical appointments, medication administration, and health monitoring.
• Support meals, transportation, community activities, and daily living tasks.
