Job summary

Sevita is seeking a Part Time Direct Support Professional to assist individuals in residential settings, delivering person-centered services per each plan. You will support daily living activities, health maintenance, and community integration while maintaining accurate records.

This role requires a high school diploma, 18+ years of age, and basic computer skills; previous human services experience is preferred. Training and supervision are provided in a supportive environment.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Deliver support per individual service plans, incl. socialization and health maintenance.
• Assist with daily activities: personal care, housekeeping, transportation, and community integration.
• Monitor progress and document activity in records; maintain confidentiality.
• Provide safe, respectful assistance and adhere to policies and procedures.
