Job summary

Heartshare in New York, NY is seeking a Direct Support Professional to implement person-centered services for individuals with developmental disabilities, supporting goals and outcomes both in programs and in the community.

You will help plan activities, assist with transportation, and promote social engagement while maintaining privacy and safety. Certifications in AMAP, CPR/First Aid, and SCIP are required or pursued as part of the role.

Qualifications

• Must meet requirements for AMAP, CPR/First Aid and SCIP certifications.
• Experience supporting individuals with developmental disabilities preferred.
• Valid NYS driver’s license preferred but not required.

Responsibilities

• Implement individuals’ plans aligned to services, goals and outcomes.
• Plan and implement activities at the program and community level.
• Support and teach individuals to develop skills and socialize.
• Ensure supports respect each person’s needs and routines.
• Promote community inclusion and maintain safety and confidentiality.
• Document information timely and accurately in required records.
