Heartshare is seeking a Day Support Professional in New York to implement person-centered services for individuals with developmental disabilities. You will help carry out individual plans, participate in community activities, and support skill development while promoting inclusion and independence.

The role emphasizes safety, accurate documentation, and collaboration with families, advocates, and providers.

Qualifications

• AMAP certification is required or in progress.
• CPR/First Aid certification is preferred.
• SCIP training is preferred.
• Strong communication and teamwork skills are essential.

Responsibilities

• Implement individuals’ plans and goals in program and community settings.
• Provide support and teach according to service goals and outcomes.
• Document information accurately and on time for medical, behavioral, and daily reports.
• Ensure safety and confidentiality of individuals and staff at all times.
