Job summary

OneWell Health Care seeks a Direct Support Professional (DSP) to provide hands-on, person-centered support to individuals with IDD in home and community settings. You will help with daily living, health, and participation in community activities.

Role requires following ISP/IPP plans, documenting care, and collaborating with an interdisciplinary team. Travel to homes and community sites is expected; evenings, weekends, and overnight shifts may be required.

Qualifications

• High school diploma or equivalent required.
• Preferred: prior caregiving, home health, or direct support for individuals with IDD.
• Experience in community-based or residential support settings preferred.

Responsibilities

• Assist with personal care and daily living activities.
• Support meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health, safety, and emotional well-being.
