Job summary

Whole Life seeks compassionate Direct Support Professionals to provide person-centered care to individuals with developmental and/or physical disabilities in Connecticut. You will support daily living activities, promote independence, and help clients engage in community and recreational activities while building trusting relationships with clients and their families.

The role includes assisting with ADLs, medications, and documenting care plans, with a focus on safety, dignity, and wellbeing.

Qualifications

• High School diploma or GED required.
• Valid driver’s license, social security card, birth certificate, or passport proof.
• Able to pass a background check, safe driving record, and abuse registry check; may require Fit for Duty and TB tests.
• Ability to stand, move, lift up to 100 lbs in some positions, and perform ADL tasks.
• Must document care in English and communicate effectively with clients and coworkers.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, meal prep.
• Administer medications and monitor vital signs; support medical appointments.
• Maintain a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities and community outings to promote independence.
• Use mobility equipment (e.g., Hoyer lifts) with proper technique and safety.
• Document daily activities, observations, and incidents accurately; maintain HIPAA confidentiality.
• Implement individualized care plans in collaboration with supervisors and family.
