Job summary

Whole Life is seeking compassionate Direct Support Professionals to join our team in Hebron, CT. DSPs provide person-centered care to adults with developmental and/or physical disabilities, supporting daily living activities, promoting independence, and encouraging community participation.

Responsibilities include assisting with ADLs, administering medications as needed, documenting observations, and coordinating with managers, providers, and families to implement individualized care plans.

Qualifications

• Required: High School diploma/ GED
• Proof of valid driver's license.
• Background check and safe driving record required.
• Ability to lift up to 100lbs and perform ADL tasks.
• Ability to document care in English and communicate with clients and coworkers.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications and monitor vital signs as needed.
• Maintain a clean, safe living environment with light housekeeping and laundry.
• Engage clients in social activities and community outings to promote independence.
• Use mobility equipment safely (e.g., Hoyer lifts) as required.
• Document daily activities, observations, and incidents with accuracy.
• Maintain HIPAA-compliant confidentiality.
• Implement individualized care plans.
