Job summary

Whole Life is seeking compassionate Direct Support Professionals to join our team in Stratford, CT. DSPs provide person-centered care to individuals with developmental, intellectual, and/or physical disabilities, assisting in daily living, promoting independence, and encouraging community involvement.

You will collaborate with managers, family members, and other professionals to implement individualized care plans, document observations, and ensure safety, dignity, and well-being at all times.

Qualifications

• High school diploma or GED required.
• Valid driver’s license and required ID documents.
• Ability to pass background checks and health screenings.
• Ability to perform ADLs and lift up to 100 lbs.
• Ability to document care in English and communicate with clients.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Administer medications, monitor vital signs, and attend medical appointments.
• Keep living areas clean and safe through light housekeeping and laundry.
• Engage clients in social activities and community outings.
• Use mobility equipment correctly and safely.
• Document daily activities, observations, and incidents.
• Maintain confidentiality per HIPAA regulations.
• Implement individualized care plans.
