Job summary

Lifeworks Services, New Hope, MN is seeking a Direct Support Professional II to advocate for disability inclusion and support individuals in community settings. The role involves creating and updating 245D plans, attending meetings with support teams, and training staff on person-centered services.

Qualifications

• Associate degree and two years in human services.
• Bachelor’s degree or 4 years education/experience preferred.
• Fluent in English, written and spoken.
• Proficiency in Microsoft Office Suite.

Responsibilities

• Advocate for disability inclusion and build rapport with individuals, their support teams, and community members.
• Create and update 245D support documents and attend meetings with support teams.
• Train staff on delivering person-centered services.
• Provide services that help individuals access the community through person-centered supports.
• Document annual plans and create SPAs with outcomes, goals, and training strategies.
• Coordinate or provide transportation to community sites as needed.
