Lifeworks Services, New Hope, MN is seeking a Direct Support Professional II to advocate for disability inclusion and support individuals in community settings. The role involves creating and updating 245D plans, attending meetings with support teams, and training staff on person-centered services.

Hours: Full-time; Monday – Friday (non-exempt)
Location: Lifeworks Services – New Hope
Compensation: $20 - $21.50 per hour
Sign on bonus: up to $1,500 (to be paid out at 60 days and 120 days of employment)
Bonus Eligible: Yearly
