Job summary

Lifeworks Services in New Hope, MN, is seeking a Direct Support Professional II to advocate for disability inclusion and coordinate community access. You will build rapport with individuals and their support teams, prepare required 245D documents, and facilitate meetings to ensure person-centered services are delivered effectively.

The role is full-time, Monday–Friday, with hourly compensation of $20–$21.50 and a sign-on bonus up to $1,500. Benefits include PTO, holidays, and a 403(b) plan.

Qualifications

  • Associate degree and two years of experience in human services required.
  • Bachelor’s degree or four years of education/experience preferred.
  • Fluent in English, written and spoken.
  • Proficiency in Microsoft Office Suite.

Position Title: Direct Support Professional II

Department: Day Services

Hours: Full-time; Monday – Friday (non-exempt)

Location: Lifeworks Services – New Hope

Compensation: $20 - $21.50 per hour

Sign on bonus: up to $1,500 (to be paid out at 60 days and 120 days of employment)

Bonus Eligible: Yearly

How you’ll be spending your time:

  • Providing services that assist people with disabilities in accessing the community through person-centered supports that lead to informed choice.
  • Documenting annual plans and creating Support Plan Addendums (SPAs), including but not limited to outcomes, goals, training strategies, evaluation plans, community resources, adaptive devices, instructional technology, therapies, and positive support transition plans.
  • Creating, implementing, and training staff to carry out outcomes and goals, training strategies, and positive support transition plans, while ensuring all required training documentation is completed as a designated coordinator.
  • Arranging and/or providing necessary transportation to and from community sites in collaboration with staff and team members, which may include driving individuals in a personal vehicle or a Lifeworks vehicle.
  • Arranging or providing additional support and assistance needed for individuals to complete personal care routines (eating, bathroom, and medical assistance), maintain and expand participation in activities, and strengthen their relationships and community involvement.
  • Building and sustaining community relationships to increase access to inclusive social opportunities for individuals.
  • Assisting individuals by collaborating with the Lifeworks employment team and other relevant services to support employment opportunities.
  • Managing billable time and ensuring all billing is completed accurately and in a timely manner.
  • Attending all required training and team meetings.
  • Following safety procedures and helping identify safety needs for oneself and others and performing other duties as assigned.

Requirements

  • Associate degree & two years of experience in the human service field.
  • Bachelor’s degree or a combination of four years of education and experience preferred.
  • Fluent in English, verbal and written.
  • Proficiency in Microsoft Office Suite.
