Job summary

Imagine the Possibilities seeks a Direct Support Professional to provide empowering, person-centered care in home and community settings across Iowa. You will implement individualized goals, assist with daily living activities, and promote independence in collaboration with the care team.

Responsibilities include supporting individuals with behavioral strategies, documenting care in Setworks, and ensuring safety and rights protections.

Responsibilities

• Collaborate with the HCBS Supervisor and Programmer to develop individualized goals and provide direct support that focuses on teaching skills, promoting independence, and encouraging participation in recreational and community activities.
• Use approved behavioral strategies to support individuals with challenging behaviors while respecting rights restrictions as outlined by the interdisciplinary team and individual service plans.
• Complete accurate, timely, and Iowa Code–compliant documentation in Setworks, ensuring all required service details, interventions, and individual responses are properly recorded and submitted daily.
• Provide hands-on support in home and community settings, including daily living assistance, transportation, mobility support, safety monitoring, property respect, and active encouragement of choice and independence.
• Administer and document medications per the six rights, follow all agency policies and procedures, maintain reliable attendance, complete required training, and communicate concerns or updates through appropriate channels.
