Job summary

Imagine the Possibilities, Inc. in Dubuque, IA is hiring Direct Support Professionals for a full-time role in a Home and Community-Based Services (HCBS) setting.

You’ll collaborate with the care team to support individuals in achieving personal goals, with emphasis on independence and community participation. Responsibilities include teaching skills, documenting care in Setworks, administering medications per rights, and ensuring safety and empathy in daily activities.

Responsibilities

• Collaborate with the HCBS Supervisor and Programmer to develop individualized goals and provide direct support that focuses on teaching skills, promoting independence, and encouraging participation in recreational and community activities.
• Use approved behavioral strategies to support individuals with challenging behaviors while respecting rights restrictions.
• Complete accurate, timely, Iowa Code–compliant documentation in Setworks daily.
• Provide hands-on support in home and community settings, including daily living, transportation, and safety monitoring.
• Administer and document medications per the six rights and maintain reliable attendance including weekends/holidays.
