Qualifications

• High school diploma required; further education in human services or a related field is a plus.
• Excellent problem-solving, and interpersonal skills. Ability to work both independently and collaboratively.
• Active participation in cultivating a ‘culture of communication’ with people we support, families, and co-workers.
• Proficiency with Microsoft applications and general computer skills.
• Maintain professionalism and positive relationships with individuals, families, colleagues, and external partners.
• Ability to lift up to 100 pounds with assistance and kneel to provide hands-on support.

Responsibilities

• Person-Centered Support: Provide individualized support based on each person’s unique skills, aspirations, and needs.
• Skill Development: Support skill-building and personal growth per PCP/ISP.
• Community Engagement: Create opportunities for community involvement and social connections.
• Daily Living Assistance: Provide coaching for ADLs and IADLs.
• Health & Safety: Ensure safe, clean living and work environments and follow care plans.
• Adaptability: Evolve approach as individuals’ needs change.
