Qualifications

• High school diploma required; further education in human services or related field is a plus.
• Excellent problem-solving and interpersonal skills.
• Ability to work independently and collaboratively.
• Proficiency with Microsoft applications and general computer skills.

Responsibilities

• Provide person-centered support based on each person’s skills and needs.
• Support skill-building and personal growth per PCP/ISP.
• Facilitate community engagement to build social networks and inclusion.
• Assist with Activities of Daily Living (ADLs) and Instrumental ADLs (IADLs).
• Ensure safe living and working environments and follow care plans.
