Job summary

Makom in Fairfax is seeking a Direct Support Professional for a Monday–Friday, 8am–5pm schedule. This role supports individuals with IDD to live self-determined lives and engage with their communities.

You will provide person-centered care, assist with daily living activities, and help build skills aligned with PCP/ISP. A focus on health and safety, adaptability, and collaborative teamwork is essential.

Qualifications

• High school diploma required; further education in human services or related field is a plus.
• Excellent problem-solving and interpersonal skills; ability to work both independently and collaboratively.
• Proficiency with Microsoft applications and general computer skills.
• Ability to lift up to 100 pounds with assistance and kneel to provide hands-on support.

Responsibilities

• Provide individualized, person-centered support based on each person’s unique skills, aspirations, and needs.
• Support skill-building and progress through PCP/ISP.
• Facilitate opportunities for individuals to engage in their communities and build social networks.
• Assist with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
• Ensure living and working environments are clean, safe, and compliant with care plans.
• Evolve approach as individuals’ needs change and remain a key member of the support team.
