Qualifications

• High school diploma required; additional human services education is a plus.
• Strong problem-solving and interpersonal skills.
• Ability to work independently and collaboratively.
• Proficient with Microsoft applications and general computer skills.
• Ability to lift up to 100 pounds with assistance.
• Maintain professionalism and positive relationships with individuals and families.

Responsibilities

• Provide individualized, person-centered support to individuals in their communities.
• Support skill development aligned with PCP/ISP.
• Foster community engagement and social connections.
• Assist with Activities of Daily Living (ADLs) and IADLs.
• Maintain clean, safe living and work environments per care plans.
• Adapt approaches as individuals' needs evolve.
