Job summary

Qualifications

• High school diploma required; further education in human services a plus.
• Excellent problem-solving and interpersonal skills.
• Ability to work both independently and collaboratively.
• Proficiency with Microsoft applications and general computer skills.
• Ability to lift up to 100 pounds with assistance.

Responsibilities

• Provide individualized support based on each person’s unique skills, aspirations, and needs.
• Help individuals lead self-directed lives in their communities.
• Support skill-building and personal growth in alignment with PCP/ISP.
• Assist with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
• Ensure living and working environments are clean, safe, and meet health care plans.
