This role offers flexible scheduling, on-site work, and hands-on community and center activities. You will assist with daily living tasks, transportation, documentation, and be part of a mission to empower independence.

Qualifications

• High school education or GED required.
• Ability to pass a background check.
• Valid driver's license and reliable transportation.
• Comfort with community-based activities and documentation.
• Physical ability to assist with transfers up to 50 lbs.

Responsabilités

• Promote community integration through planned activities.
• Provide personalized support and daily living assistance.
• Arrange transportation and accompany clients to appointments.
• Support medical care including scheduling and basic administration of meds.
• Document progress and maintain records.
• Maintain flexible scheduling to meet client needs.
