Qualifications

• Active and engaged in community activities.
• Tech-savvy with basic computer tasks and documentation.
• Background check ready without issues.
• Reliable transportation and valid license with insurance.
• HS diploma or GED; prior relevant experience a bonus.

Responsibilities

• Promote Community Integration: plan and facilitate activities for social development and inclusion.
• Provide Personalized Support: assist with hygiene, meals, and household tasks.
• Transportation and Accompaniment: provide transport and accompany to appointments and events.
• Medical Assistance: support medical care including scheduling and attending appointments.
• Skill Development: train in life skills like communication and daily living activities.
• Document Progress: maintain progress notes and activity logs.
