Job summary

Hope Network / New Center is seeking a Direct Support Professional (DSP) for 2nd shift, full-time (32 hours). The role focuses on helping individuals achieve independence and community integration in Michigan.

The DSP provides personal care support, monitors safety, assists with daily living activities, and collaborates with care teams to implement care plans while encouraging community participation.

Qualifications

• High school diploma or GED required.
• Valid State of Michigan driver's license required.
• Ability to lift 50 lbs. required.
• Ability to become CPR/First Aid certified.
• Commitment to scheduled shifts, including weekends and holidays.
• Possess basic computer skills.

Responsibilities

• Monitor and ensure safety 24/7 in individual care settings.
• Support crisis prevention, intervention, and resolution.
• Establish collaborative relationships with individuals served and care teams.
• Foster partnership with individuals served, the community, and professionals.
• Help build skills for problem-solving, communication, and independence.
• Assist individuals with social activities and daily living tasks.
• Monitor medical vitals and relay information between providers and care team.
• Understand and assist in implementing care plans and document accordingly.
• Connect individuals with community opportunities and employment or education goals.
