Job summary

Homes-For-Independent-Living seeks a compassionate direct care professional to support individuals with special needs in a community setting. You will assist with daily living activities, implement ISP/ITP and BSP plans, and monitor progress while documenting health and behavior concerns to ensure safety.

You will collaborate with supervisors and care teams, transport clients to appointments, and help create engaging programs that promote independence and community involvement.

Qualifications

• Experience in direct care and assisting with activities of daily living.
• Ability to document progress and incidents per policy.
• Strong communication with team and other stakeholders.

Responsibilities

• Provide direct care and supervision of individuals served.
• Assist with ISPs/ITPs and BSPs and program routines.
• Monitor and document progress, behaviors, health concerns and incidents.
• Safely transport individuals to appointments and community activities.
