Job summary

Homes-For-Independent-Living is seeking compassionate direct care professionals to support individuals with special needs, ensuring health, safety, and well-being through ISP/ITP and BSP planning.

You will assist with daily living activities, facilitate skill development and community engagement, document progress, and collaborate with the care team while maintaining a safe, welcoming environment.

Responsibilities

• Provide direct care to and supervision of individuals served, ensuring their health, safety, and well-being.
• Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
• Facilitate activities that promote skill development, independence, recreation, and community engagement.
• Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.
• Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
• Safely transport individuals to appointments, activities, or community outings as required.
• Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements.
