Qualifications

• MUST PASS AZ DPS Level 1 Fingerprint Clearance application & Criminal History check
• Valid Arizona Driver’s License with clean driving history (Willing to transport members)
• CPR & First Aid Certification (Preferred)
• Article 9 Certificate (Preferred)
• Direct Care Worker Certificate (Preferred)
• Prevention & Support Certificate
• 18+ years of age

Responsibilities

• Respite (RSP): supervise and care for Tyler’s Place AZ members' physical, mental and/or emotional wellbeing in the member's home or in their community while relieving a family member or person caring for that member.
• Attendant Care (ATC): assist Tyler’s Place AZ member in creating/maintaining a safe, clean-living environment which may/may not include personal cleanliness. Provider will also assist member to engage in community outings and/or activities and to help develop and maintain a daily schedule and/or daily plan of care.
• Habilitation (HAH): assist Tyler’s Place AZ member in a variety of interventions based on the member's ISP goals to help them become more independent by improving their daily life and to be a valued member of his/her community. Interventions may/may not include Habilitation therapies, behavior interventions and/or sensory motor implantation.

Tyler's Place AZ Inc. is a family-owned agency that is based out of Chandler, AZ that is licensed throughout the entire state of Arizona. As a rapidly growing agency we are currently seeking providers to work with individuals with developmental disabilities in the home and the community. As a Direct Care Worker, you will be aiding and support to all age groups by helping to meet their individual goals. We work in a fun and easy-going environment while maintaining a strong work ethic.
