Tyler's Place AZ Inc. is a family-owned agency that is based out of Chandler, AZ that is licensed throughout the entire state of Arizona. As a rapidly growing agency we are currently seeking providers to work with individuals with developmental disabilities in the home and the community. As a Direct Care Worker, you will be aiding and support to all age groups by helping to meet their individual goals. We work in a fun and easy-going environment while maintaining a strong work ethic. We are currently looking to add to this dynamic team.
Respite (RSP): The qualified person will supervise and care for Tyler's Place AZ members physical, mental and/or emotional wellbeing in the members home or in their community while relieving a family member or person caring for that member.
Attendant Care (ATC): The qualified person will assist Tyler's Place AZ member in creating/maintaining a safe, clean-living environment which may/may not include personal cleanliness. Provider will also assist member to engage in community outings and/or activities and to help develop and maintain a daily schedule and/or daily plan of care.
Habilitation (HAH): The qualified person will assist Tyler's Place AZ member in a variety of interventions based on the members specific ISP goals to help them become more independent by improving their daily life and to be a valued member of his/her community. Interventions may/may not include Habilitation therapies, behavior interventions and/or sensory motor implantation.
